Catastrophic Illness - Application for Donation of Leave" is the form needed if you have been classified as having a catastrophic illness or injury, have exhausted all accumulated leave (VA, SP and CT), and wish to receive donated leave.
Catastrophic Leave - Transfer Authorization" is the form needed if you wish to donate accumulated leave to an employee who has applied for donation of leave for catastrophic illness.
Change Request - is the form needed if an employee wishes to change their name or contact information that is on file with the Personnel Department. Please note that a name change request must be accompanied by a copy of the new social security card.
Classification/Position Action - is the form needed for Appointing Authorities to request the creation of a new classification, amendment of an existing classification, creation of additional positions for classifications which already exist within the jurisdiction, amendment of a salary grade, or other classification or position actions within the Mobile County Merit System.
Educational Incentive Pay Request - is the information and form used by employees who are eligible for Educational Incentive Pay. Please read the Educational Incentive Pay Request Information carefully before submitting your request. Note: An official transcript from an accredited college or university must be submitted to the Mobile County Personnel Department for review of the Educational Incentive Pay request. An accredited college or university is one which is accredited by a regional accrediting body or the Distance Education Accrediting Commission (DEAC). Employment Separation Request - is the form used by the jurisdictions to notify us of any changes in retirements or resignations.
Pre Disciplinary Action Notice - is the form needed to notify an employee of possible disciplinary action. For more information regarding this form, please contact (251) 445-4584. For additional information on the disciplinary process, specifically dismissal, suspension, or demotion, see Rule 14.
Request for Injured with Pay - is the form needed if you wish to request a leave of absence with pay, due to injury incurred while performing the duties of your position.
Request for Absence with Leave without Pay - is the form needed if you wish to request a leave of absence without pay. *NEW* If this absence is due to a Worker's Compensation claim, you may check the box at the top of the form, and include a copy of the first report of injury.
Military Leave Form - is the form needed to request or return from a leave of absence for military training or active duty.
Request for Maternity Leave - is the form needed if you wish to request a leave of absence for maternity issues. Please see the form for additional details.
Request for Reinstatement (after resignation or pension) - if you are requesting to be placed on a list after resignation or pension, please click here: Re-Employment Opps Page to submit your request.
Request to Change Employment Options - is the form needed if you are active on a current Employment Register or Re-Employment List and wish to change: a) your status from active to inactive; b) the type of employment you will accept; c) the jurisdictions where you will accept employment (Click here for instructions).
