Choose the appropriate form by clicking on the image to the right.

Please review the instructions below.

Open to all Merit System Employees.  If you wish to enroll in a class:
1.   Select from the programs listed on the training schedule/summary.
2.   Complete a “Training Request” form. 
3.   Obtain the necessary approvals.
4.   You may return the form in person to the MCPB/ Training Office, by FAX TO 470-7722 or email your form (with supervisory approval affixed) to training@personnelboard.org
